Thomas W. "Tom" Perkins, 74, of Shadyside, died July 7, 2021.

He was born October 15, 1946, in Armstrongs Mills, Ohio, a son of the late Clyde Perkins and Rosalee Keating Perkins.

Tom was a coal miner and owner and operator of T&A Contracting. He and his wife, Alice are members of Hillcrest Bible Church. Tom enjoyed fishing and hunting.

In addition to his parents, he was preceded in death by his son, Jeff Perkins; a sister, Betty McClelland; and a brother, Clyde "Bud" Perkins.

Tom is survived by his loving wife of 29 years, Alice Mae Glass Perkins; a son, Tony Perkins; three stepsons, Keith Otto, Troy Otto and Scott Otto; a brother, Jay Perkins; seven grandchildren; and nine great grandchildren.

Private Services were held at the convenience of the Perkins family. Arrangements were entrusted to Toothman Funeral Home in Jacobsburg, Ohio.
