The goods and services shown below are those we can
provide to our customers. You may choose only the items you desire.
However, any funeral arrangements you select will include our basic
services and overhead. If legal or other requirements mean you must buy
any items you did not specifically ask for, we will explain the reason to
you in writing on the statement we will provide to you describing the
funeral goods and services you selected. This list does not include
prices for certain items that you may ask us to purchase for you, such as
cemetery or crematory services, certified copies of death certificate,
clergy honorariums, flowers, cemetery monument lettering and newspaper
notices. The price for those items will be shown on your bill or the
statement describing the funeral goods and services you selected. We may
charge you for our services in buying these items.
ALTERNATIVE SELECTION PACKAGES
Offering A: Traditional Full Service Package, with one or two days of
visitation and funeral. $

Other preparation of the body; Use of facilities and staff for viewing,
one or two days, at the funeral home or elsewhere; Use of facilities and

alternative container; transportation to crematory; necessary
authorizations; and the cremation. The containers we provide are
fiberboard containers or unfinished wood boxes.
Offering H: Belmont Co. Burial Award Services
$ 1,500.00
Eligibility requirements must be confirmed through
Human Services. Basic services of funeral director and staff and overhead; Local
transfer of remains to funeral home; All necessary preparations of the
body; Hearse or other necessary motor equipment; Minimum casket or the
Crematory fees; Filing of all the necessary paperwork, permits and
certificates. All Items Discounted.
All Infant and children services are provided at no
charge to the family.

Complete itemization of
services and merchandise with a description and prices for those items.
Basic Services of Funeral Director and Staff and Overhead $
1,490.00
Our services include: conducting the arrangements conference; planning the funeral
or other services; consulting with family and clergy; shelter of
remains; preparing
and filing of necessary notices; obtaining the necessary authorizations
and permits;
coordinating with cemetery, crematory, or other third parties. In
addition, this fee
includes a proportionate share of our basic overhead costs.
This fee for our basic services and overhead will be added to the total
cost of the
funeral arrangements you select. (This fee is already included in our
charges for
direct cremations, immediate burials and forwarding or receiving remains
and any of our service packages.)
Embalming $
725.00
Except in certain special cases, embalming is not required by law.
Embalming may
be necessary, however, if you select certain funeral arrangements, such as
a funeral
with viewing. If you do not want embalming, you usually have the right to
choose
an arrangement that does not require you to pay for it, such as direct
cremation or
immediate burial.
Other Preparation of the Body $

Transfer of Remains to Funeral Home $

Includes local removal within a 35 mile radius. Beyond local
removal radius

Our charge for a direct cremation (without ceremony) includes: basic
services of
funeral director and staff; a proportionate share of overhead costs;
local removal
of remains; transportation to crematory; necessary authorizations; and
cremation.
If you want to arrange a direct cremation, you can use an alternative
container.
Alternative containers encase the body and can be made of materials like
fiberboard
or composition materials (with or without and outside covering). The
containers
we provide are a fiberboard container or an unfinished wood box.
